Health & Exercise Consent
The confidential lifestyle questionnaire will enable Lewis Bailey to supplement any
G.P or medical professional’s activity programme you may be undertaking. You agree, you consider
yourself fit to follow a course of exercise and nutritional advice. You are aware that
it is advisable to consult your GP or therapist prior to following a new training
regime or course of therapy, and by agreeing to this form you choose not to obtain
a GP or therapist's examination and undertake all activities and guidance at your own risk.
The above information is correct to the best of your knowledge, and you agree to
notify Lewis Bailey of any change in your health before progressing further in the programme.
Terms of payments
Lewis Bailey charges a monthly subscription payment for this product. If you cancel your subscription, you will no longer have access to digital content.
You understand that Lewis Bailey operates a no refund policy.
In the event of unforeseen circumstances, a credit note will be given for the
package to be used either at a later date or by someone else of your choosing.
You also understand that should you cease using Lewis Bailey’s services, you’re not entitled to any refund.
Holidays / Sickness / Unforeseen circumstances
There may be ad hoc occasions when Lewis Bailey will need to re-arrange coaching sessions, and you will be notified immediately.
Your right to cancel
You have the right to cancel this agreement at any time. Simply log in to your account on the Lewis Bailey website and cancel the subscription.
You have a cooling period of 14 days in which to cancel your account and receive your refund. During this 14
period Lewis Bailey will deduct any bank charges incurred from your refund.
You waive the right to a refund during the cooling-off period should you
download, consume or access any of the content or participate in any
coaching call or training session with Lewis.
